Incident reporting of Controlled Drugs (CDs)
Incidents/concerns involving CDs need to be reported via the web-based CD reporting tool. This tool makes reporting much simpler and consistent and trends can be identified more easily with learning from incidents shared in a timely manner.
The CD online tool is ‘live’ across the Country and organisations are required to record their concerns via the following website address:
In order to report CD incidents/concerns you will need to register on the website and as part of your registration process the region that should be selected is Midlands.
Once logged onto the reporting tool the following options are available:
- Report an incident/concern
- Request a witness for destruction of CDs
- Submit an Annual Declaration - currently this function is disabled for the North Midlands
- Submit a Quarterly occurrence report
- Export data
Any submission that is made via the tool generates an automatic email notification to Sam Travis as CDAO and the CD team. The submitted report is also saved in the reporters domain area with a unique reference number for future reference
